How we estimate this
  • Insurers on HealthCare.gov denied 19% of in-network claims in 2024. Fewer than 1% of denied claims were appealed. (KFF, 2024)
  • Claim counts start from typical yearly use by age group. Each doctor visit, medicine refill, and procedure creates its own claims.
  • People taking 5 or more medicines had about 48% more office-visit costs in a national study of older adults. (MEPS analysis)
  • Missed doses are linked to more urgent care and hospital stays, which means more claims.
  • Shift work, physical labor, and hazardous exposures raise injury and chronic illness risk (NIOSH). Injuries covered by workers' comp aren't counted here.
  • Savings per approved appeal = the claim's average allowed amount × 80%, the typical share a plan covers. If a claim is denied, you owe all of it. If the appeal wins, your plan pays its share. Allowed amounts come from national survey data: $308 per office visit, $233 per prescription fill, and $1,344 per hospital outpatient visit. (AHRQ MEPS, 2024) Therapy uses $168 per session, the average rate for in-network therapists. (JAMA Network Open, 2024)
  • Storage is valued at $12 a year, the price of Apple's iCloud+ 50GB plan, the cheapest common paid storage plan.
  • Unlimited chat only counts in months your household would go past free Leo's 50 messages. We assume about 4 messages for each visit, checkup, therapy session, or denial, spread across the year, plus about 30 more in any month with a procedure or an appeal. Each month over the cap is valued at one month of LeoPlus, $9.99.
  • We assume half of appeals succeed. LeoPlus includes 3 appeal letters a quarter, up to 12 a year. Free Leo doesn't include appeal letters. We apply the 12 letters to your largest denials first, and we assume every household has at least 1 denial worth appealing.

This is an estimate, not a promise. Every plan and every case is different, and no one can guarantee an insurer's decision.
